About Flock

Flock Health has built a patient and community app that predicts acute mental health crises for patients with a serious mental illness (SMI) and an integrated telehealth clinic for providing clinical interventions. Through this system of care, we’re able to stabilize patients quickly, preventing the need for hospital visits while providing safety and peace of mind for patients and their loved ones.

Our team has deep personal and professional experience with complex behavioral health conditions. We have come together from diverse backgrounds because we believe that care needs to be dramatically improved for patients living with serious mental illness, and our team is passionate about making this a reality. At Flock, our values are People, Integrity, Caring and Excellence, and we actually, really care about our patients getting healthier and staying healthier.

About You

We are looking for an LCSW who is passionate about improving care for the SMI population, ideally with several years of experience in this domain (likely in a higher acuity setting – inpatient, locked facility, residential or partial programs). You should be comfortable clinically evaluating SMI patients and triaging next steps based on your assessment.

Our ideal candidate will also have a strong desire for innovation, as they will be integral in helping to build and refine our product. They will feel comfortable interfacing between our business and product teams, and will be able to precisely articulate how our product and service can be improved.

About the Role

As a Social Worker/Program Builder at Flock, you will be both a frontline clinician, carrying out the program’s care model (with the support of our medical director and prescribing clinicians), and a program builder, contributing to our technology and operations. Responsibilities will include:

  • Frontline Evaluation and Triage: You will conduct mental health evaluations, develop an assessment and triage accordingly
  • Direct Clinical Care: You will provide psychotherapy and other appropriate clinical interventions, provide regular check-ins, and help patients better learn how to manage their care
  • Crisis Navigation: You will help patients in crisis navigate the complex mental healthcare system, including liaising with ER staff and other provider partners
  • Innovation: You will work directly with the team to improve our care model, our tech interface, and our internal operations. We’ll be actively seeking your input and will ask you to leverage your expertise to identify areas for improvement.

Skills + Competencies:

  • Licensed as a LCSW or LMFT in the state of California
  • 5-10 years of direct experience caring for patients with complex behavioral health concerns
  • Ability to clinically evaluate a patient’s mental health condition, assess their status, and determine their degree of decompensation
  • Strong experience and comfort with mental health triage, particularly with SMI patients and more severe or urgent circumstances
  • Strong interpersonal communication skills and comfort collaborating with both clinical and non-clinical team members
  • Passionate about novel models of care and building them
  • Some operational and/or people management experience preferred
